If you are an internal staff member who will be submitting requests on your own behalf (i.e. not for a group, team, club, etc), use your first and last name as the Group Name.

Additional Group Information

External / Outside Groups: Upload the Group's Certificate of Insurance and its Expiration Date.

Upload your insurance certificate (PDF or JPG)

Enter expiration date in MM/DD/YYYY format

All items must be received for Application to be Approved
  • District to be listed as additional insured (CG2026 Endorsement) or other endorsement attached
  • The Owner of the Insurance Policy and the Organization requesting use must be identical
  • Minimum $1,000,000 per Occurrence; $2,000,000 Aggregate
  • Rated “secured” by AM Best
  • NY State Admitted
  • 30-day Cancellation
  • Primary Coverage for the District, the Board of Education, District Employees and Volunteers
  • District indemnified for deductibles
